Rochester Periodontal Group First Visit

What Will Happen On My First Visit?

We will examine your teeth and their supporting structures, including the gums and bone, in the area(s) of concern.  This may include taking any needed X-rays or impressions.  Depending upon how complicated your situation is, we may be able to provide you with suggestions for treatment at that time, or may need to study the information we get to provide you with treatment alternatives at a later time.

• Your referral slip and x-rays from your referring dentist if you received either of those.
• A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website).
• If you have dental insurance, please bring any insurance cards with you to the appointment.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
